1. Rates are discounted in exchange for a released liability value for cargo claims of $2.00 per pound maximum. 2. Payment Terms are NET 15 Days. 3. Rate quotes expire in 90 days unless otherwise specified. 4. Customer is responsible for loading and/or unloading. Pump / Compressor fees are not included in this rate. 5. Maximum loading and unloading time is 1 hour each for FTL, ½ hour each for LTL, and 2 hours each for Bulk, after which additional charges will apply. 6. Where applicable, shipments must be blocked and braced by Shipper in compliance with applicable regulations. 7. All paperwork including Customs documents and terms of sale or payment of goods remain the responsibility of the shipper and/or consignee. 8. Additional cargo insurance is available upon written request. 9. Pallet or skid quotations are based on a standard skid size of 4'x4'x6', weight not exceeding 1800 lbs. 10. Feet of trailer or lbs quotations are based on 900 lbs per linear foot of trailer. 11. Freight class and weight subject to inspection. Freight charges are to be collected according to proper description. 12. All freight claims and other transportation related liabilities are the direct responsibility of the transport carrier. 13. Extra charges shall be assessed for the services beyond specified Terms and Conditions. 14. All trans border shipments are subject to a border crossing surcharge. 15. All inter modal rates and shipments quoted are subject to the requirements of The Inter modal Safe Container Act. 16. Rates are subject to DSN standard accessorial charges 17. Rates only apply to for the specific equipment type quoted. 18. Rates are based on equipment availability. 19. These terms and conditions will apply to all shipments unless otherwise specified in writing prior to transportation of goods.
I hereby apply for a 15 day account with DSN. I understand the terms of payment on this account are Net 15 days and I agree to pay the account on that the information given herein is true and correct, and I understand that it is being used to determine my credit responsibility. DSN is authorized to obtain any information it may require relative to this application from any sources to which DSN may deem necessary and each such source is hereby authorized to provide DSN with such information. DSN is furthermore authorized to disclose, in response to direct inquiries from any other lender or any Credit Bureau, such information on my account as DSN considers appropriate, and I agree to indemnity DSN against and save DSN harmless from any and all claims in damages or otherwise arising from such disclosure on DSN's part. DSN is also authorized to retain this application. I also understand that DSN's Accessorial charges are an inherent part of DSN's billing process and agree to above accessorial charges for additional services rendered. These charges constitute an a addition to any quotation submitted by DSN. These charges will apply to any and all DSN shipments unless DSN has specifically agreed in writing to include the specific charge in the rate quotation prior to pick up of the shipment.
